Answers to 24 Hour Emergency Alert Questions... Q. What does the Emergency Alert Program cost? A. The Emergency Alert Program is just on of the benefits included in your benefit program with ILSA. When your benefit program expires, you may renew all of the benefits in your membership package for an entire year for one low cost! Other benefits included in this program include prescription and vision care benefits. Q. What will it cost to enroll my spouse and dependents in the Emergency Alert Program? A. Each additional member can be added for as little as $14.95. Q. Who has access to my Emergency Alert information? A. This information is relayed to emergency personnel only when the need arises (your signature on the emergency alert data sheet authorizes us to do this). Any information we have on file for you has restricted access and is protected. Q. What if I lose my Emergency Alert Card or if my yellow Emergency Alert Sticker wears off my Driver's License? A. Contact ILSA's customer service associates. They will promptly send you replacement materials! The toll-free number is 800-895-5535. Q. What happens if I do not send in my Emergency Alert data sheet? A. The Emergency Alert benefit is not activated. You may, however, continue to use the other benefits ILSA offers. Q. Do I have to complete a new Emergency Alert Data Sheet each time I renew? A. Yes. It is important to keep this information current. Also, your signature on the emergency alert data sheet authorizes us to release information only during your membership period. Answers to Rx Savings Questions... Q. What does calculated Co-pay mean? A. This means that although you pay the full cost of your prescription to the pharmacy. The processor calculates the amount electronically. Your price includes a discount off the cost of the drug and the pharmacist's dispensing fee. This negotiated price is generally lower than the pharmacy's regular price. Q. What happens if my pharmacy's regular price is lower than the negotiated price? A.The pharmacist should submit the regular price to the processor. The processor allows only the lower of the regular price or the negotiated price. (It is not uncommon for some stores to advertise "loss leader drugs" - drugs that they are willing to lose money on to encourage customers to come to their pharmacy.) If your drug is advertised at a lower price than the negotiated price the pharmacist should submit the advertised price to the processor. Processing is needed for safety checks of your drug profile. Q. How can I tell if my prescription has been transmitted? A. A prescription that has been electronically processed should have a 14-digit "Claim Reference Number" printed on your receipt or label. Always check for this number. This way you can be assured that you have received the lowest price and that your medication has been checked against your central profile for problems. Your pharmacist should be glad to show you where this number appears on your receipt or label. Q. Do I need to send my pharmacy receipts to anyone? A. No, your prescription is automatically validated and checked against your profile. Q. What pharmacy can I go to? A. This plan recognizes Any Willing Pharmacy Provider. Any pharmacy that agrees to meet the plan's conditions and electronically submit your prescription can fill it for you. Thousands of pharmacies across the USA have already agreed to do this. However, if your personal pharmacy is not a member of this network, the pharmacist can call the toll-free number on your membership card and immediately arrange to process your prescriptions. |

Q. What can I do if a pharmacy will not honor my drug card? A.Pharmacy is an increasingly competitive business - as is all of health care. However, some may not wish to compete in today's environment - or they may feel that they do not need to. This plan gives any pharmacy desiring to participate the opportunity to do so. If you wish to take advantage of your plan and its benefits, you will need to go to a pharmacy that will participate. This may require that you use a new pharmacy. Fortunately there are many who value you as a customer. Remember, a decision not to participate is that of the individual pharmacy, not the plan. Q. Can I transfer my prescriptions to a pharmacy that will honor my card? A. Yes. The pharmacist at the store you wish to transfer your prescription to can usually do this for you. Q. What prescription quantity can I get? A. This is determined by your physician. This plan does not limit quantities. Generally, you will get a better price at the pharmacy for larger amounts. If yours is a maintenance drug (one that you will be taking for a long time) you might ask your doctor to prescribe the most economical amount that is safe for you. Many factors determine the quantity of drug to prescribe - you should discuss these with your doctor. Answers to EyeMed questions... Q. Where can I use the EyeMed Card? A.You can use your EyeMed Card at nationwide chains such as Sears, JC Penny, and other large chain locations. Call us and we will tell you who's in your area. Q. How often can I use the EyeMed Card? A. You and your family members may use the EyeMed card as many times as you wish. There are no limits on the number of times the card can be used or a cap on savings. Q. Can my family get eye care discounts? A. Each family member you add to your membership will be provided the EyeMed benefit program. Answers to general questions... Q. Do you give or sell my name, address or other information to any other company? A. Absolutely not! We maintain the confidentiality of all our members and do not sell, provide or publish our member database to outside sources. Q. How do I know when my membership expires? A.Your benefit cards will have an expiration date printed on them. Also, if there is any question of when your benefits expire, just contact us. We are happy to answer any questions you may have about your membership status or the benefits we offer. We send all of our members a renewal notice to let them know their savings program is about to expire. Q. How do I renew my membership? A.You may renew your membership a number of ways...
The annual membership fees are: Individual membership - $25, additional family members (spouse/dependents) - $14.95 each (with a maximum charge of four additional members, no matter how many members are added). Discounts may vary. |
